What the site collects

When you send a message, the form collects your name, email, phone number, how and when you would like us to reach you, and the message you write. It does not ask for health details, and we ask you not to include them.

How we use it

We use what you send to call or write back, and to schedule a call or a visit. We do not sell it.

Your records

We keep your records private and share them only as the law allows, or when you ask us to. Intake forms and your plan of care live in our own patient portal, which has its own privacy terms.

Booking and links

Booking opens a page run by our scheduling provider, and what you enter there is covered by that provider's own policy. Links to other sites are covered by their own policies.
